Actions plans specify:

1. Objectives to be achieved including contribution to overall strategic objectives;
   Objectives in the Action Plan are SMART
   • Specific
   • Measurable
   • Achievable
   • Realistic
   • Trackable
2. Actions needed to achieve each key outcomes and associated outcomes;
3. Who will complete each action (responsibility and accountability);
4. When the outcomes will be achieved;
5. Links between major outcomes and organisational objectives;
6. Measurement criteria to evaluate the results achieved;
7. Other relevant information.
